Where things stand

Trokos is an Australian developer of clinical-reasoning software for licensed musculoskeletal clinicians. No Trokos software is currently supplied in the United States. Trokos has made a regulatory-status determination and is preparing a request for the U.S. Food and Drug Administration’s feedback on it; no software will be made available in the United States until that process is complete. Special tests, and what the evidence actually says

Musculoskeletal assessment leans heavily on special tests, and their value in telling one pathology from another varies a great deal. Some discriminate well. Others carry a reputation their evidence does not support. For most, the underlying work is scattered across decades of primary studies that no clinician in a full caseload has time to read, and the summaries that reach us second-hand rarely say how strong the original finding was.

Trokos is built on a simple position: a clinician should be able to see which published work sits behind any weighting, and judge that work themselves. Every finding a tool acts on carries its source. Where the evidence is thin, the tool says so rather than presenting a confident answer — because a clinician who cannot see the strength of the reasoning cannot properly disagree with it.

That is a statement of how the tools are constructed, not a claim about how well they perform. Measured performance is a separate question, and we will publish it — including the unflattering parts — before any tool is made available.

How Trokos tools are built

Each tool is a single file that runs entirely in the clinician’s browser. There is no server, no account that holds patient information, and no network call during use. The reasoning is deterministic: the same inputs always produce the same output, and every step of it is inspectable — the file can be opened in a text editor and the weights and their sources read directly.

A tool that supports a clinician’s judgement has to be honest about how often it is wrong, and about how much is not yet known.

Who this is for — and who it is not for

Trokos tools are for licensed clinicians who are competent to perform and interpret the assessment independently of the software. They support clinical reasoning; they do not replace it. They are not for patients or members of the public, they do not diagnose, and they do not recommend treatment.
